A List of Documents which are enclosed with the application of BAS registration of a foreign manufacture on the territory of Ukraine.

1. The Power of Attorney.
2. The code of External Economic Activity from Chamber of Commerce and Industry.
3. Explanatory note, which includes information  about the name, structure and purpose of food supplement, its special pecularities  and producer’s recommendations concerning product’s destination.
4. Official documents from the producer, which prove the obligatory operation factors of safety, minimal specifications of quality, information as for specific action of a product ( certificates, declarations of a producer, registration license, registration facts in the producer’s country ) – for the production which is imported.
5. Text of marking of the food product (in Ukrainian language).
6. Scientific reading, results of testing from the certificated establishments which conducted the tests.
